Key Responsibilities:
- Telehealth Visits: Conduct real-time video visits and follow-up consultations for comprehensive healthcare services, including chronic care management, remote patient monitoring, and urgent care needs.
- Care Coordination: Collaborate effectively with our internal care team and partner care teams (physicians, nurses, dietitians, behavioral health specialists, and health coaches) to craft personalized and comprehensive care plans.
- Patient Education & Support: Empower patients and caregivers with health education and counseling, guiding them through treatment options and effective condition management.
- Clinical Documentation: Maintain meticulous and up-to-date electronic health records (EHR), ensuring accuracy in patient histories, treatment plans, and follow-up recommendations.
- Prescription Management: Evaluate patient needs thoroughly and prescribe medications, including controlled substances, in full compliance with DEA regulations.
- Quality & Compliance: Adhere strictly to clinical protocols, best practices, and relevant regulatory standards to uphold the highest quality of care.

📌 Requirements
- Licensure: Active and unrestricted Physician Assistant or Nurse Practitioner license in the State of New Hampshire (required). DEA license (strongly preferred). Applicants without an active and unrestricted PA or NP license will not be considered.
- Education: Master’s or Doctoral degree from an accredited Nurse Practitioner program or NCCPA-certified PA-C (required).
- Certification: Board-certified as a Nurse Practitioner (e.g., ANCC or AANP) in a relevant specialty (Family, Adult, Geriatric, etc.) (strongly preferred).
- Experience: Minimum of 1 year of clinical experience practicing independently as a Nurse Practitioner or Physician Assistant (required). Telehealth or virtual care experience, as well as experience in obesity medicine, nutrition, and lifestyle modification, are preferred. Experience with Medicare and Medicaid care delivery is also preferred.
- Technical Skills: Comfortable and proficient in using telehealth platforms, EHRs, Google Workspace (e.g., Gmail, Docs, Sheets), or similar software tools.
- Other Requirements:
- Ability to manage patient interactions effectively via video and phone.
- Excellent communication, problem-solving, and organizational skills.
- A reliable internet connection and private, HIPAA-compliant workspace at home.
